HealtheIntentSM is Cerner’s population health platform, which has the unique capability to aggregate and normalize massive amounts of data across vendors and platforms to construct the longitudinal record for the person.
HealtheIntent is open to support all different types of population health management activities, such as community based care management, registries and measures and population health analytics. Cerner intelligence encodes data from internal and external sources to deliver proactive recommendations and focused information into the clinicians’ workflows. Open and interoperable technology connects data in ways that are required to support a value-based care model.
APIs for population health data
Cerner Ignite APIs for HealtheIntent allow you to deliver population health information to clinicians, administrators and analysts regardless of the system they are working in. Through use of HealtheIntent APIs you are able to expose the patient’s longitudinal record and other outcomes from across the ecosystem and present that information within the workflow.
These are the same APIs used to integrate our pop health data into the Cerner EHR platform!
Memorial Hermann has already started using the Ignite APIs. For Memorial Hermann affiliated physician practices that use the eClinicalWorks EHR, the Cerner Ignite APIs for HealtheIntent enable direct integration of Cerner HealtheRegistries registries and measures into the eClinicalWorks clinical workflow.
HealtheIntent API access
If your organization owns a HealtheIntentSMsolution, then you own the APIs that go with it! Developers who are working on behalf of a HealtheIntent client are also able to work with these APIs.

Data Syndication is the bulk delivery of sets of longitudinal person records. It provides direct, low-level, asynchronous access to the information that HealtheIntent solutions create, curate, and operate against. This API is used mainly to populate a data warehouse or other third-party data store for research, reporting, or analytical activities.
The HealtheRecord API provides data for a person’s longitudinal record including: allergies, conditions, demographics, medications, procedures, vaccinations, HealtheIntent ID lookup, person search.
The HealtheRegistries API provides structured JSON representation of a variety of data sources:
- Person Registries – Allows retrieving information about a persons’ registry data.
- Person Search – Allows searching for people based on specific search criteria.
- Supporting Facts – Allows retrieving all of a person’s supporting fact data for a provided measure within a program.
- Program Reference Data – Allows retrieving reference data about programs and measures.
Hierarchical Condition Category (HCC) is a risk adjustment payment model mandated by the Centers for Medicare and Medicaid Services (CMS). There are more than 9000 ICD-10 codes that map to 70+ HCC codes in the model. CMS requires documentation of the person’s condition at least once each calendar year to ensure reimbursement.
Preventable readmissions are extremely costly to the health care industry. Cerner’s readmission risk algorithm considers several risk factors to determine a person’s Readmission Risk Score representing the likelihood of readmission for the person.
