The Healthwise® FHIR® Solution for Cerner: Point of Care app gives physicians, hospitals, and health systems easy access to a robust collection of patient education inside Millennium, including videos, patient instructions, and shared decision making tools.

Delivering materials to the patient is easier than ever with options for printing at the point of care or digital delivery to an email address or the HealtheLife™ Patient Portal. Patients enjoy receiving multimedia communications that are compatible with mobile devices.

Built into the app, a comprehensive analytics platform provides actionable insights into patients’ engagement with education resources. Analytics help you identify opportunities to enhance clinician-patient communication to achieve better outcomes and care experiences.

Use Cases

  • physician assistant treating a patient for heart failure prescribes ACE inhibitors. He quickly accesses information on safely using the medicine and provides it to the patient during the visit.
  • nurse practitioner conducts a follow-up appointment with a patient who has multiple complications and comorbidities. She checks what education material was previously given to the patient and whether it was viewed. Then she has an informed conversation with the patient about their care journey before providing new content based on the patient’s needs.
  • physician works with a patient who is weighing different treatment paths for hip pain and shares a decision aid to find out whether the patient wants to consider hip replacement as an option. After completing the decision aid, the patient’s stated priorities and preferences are captured in the medical record, so the physician better understands what’s important to the patient.

Patient data from PowerChart—including demographics, diagnosis, medicines, and procedural codes—triggers a list of relevant education materials clinicians can access within Cerner Millennium. Clinicians can choose content in multiple languages and a variety of formats, including videos and interactive tools.

Personalize Education for Each Patient

Edit the information to include special instructions or custom content for your patient before printing or sending it to the patient portal.

Favorites Tab

Quickly access frequently used education by saving it as a Favorite. Find education on other diagnoses or medications by starting a new search, browsing through categories of medical topics, or choosing from an A–Z list.

Documentation

Education given to your patient is documented in the patient record, which allows the entire care team to see what each patient received and to use for quality measures.

Report on Patient Activity and Responses

View your patient’s activity in the Patient Response tab. See your patient’s engagement with the education materials, and view details on stated preferences and level of understanding of treatment options. Captured responses help guide the next step in the care plan.
