UpToDate® Pathways

With the UpToDate® Pathways FHIR® app, clinicians can reduce unwanted care variations with evidence-based, patient-specific interactive algorithms targeting common, chronic, and complex conditions that have multiple management, diagnosis, or treatment options. Applicable to United States only.

With the UpToDate® Pathways FHIR® app, clinicians can tailor patient recommendations at the point of care via interactive algorithms created and maintained using UpToDate’s rigorous editorial process and network of more than 6,700 physician authors, editors, and peer reviewers. Each pathway leverages the breadth of knowledge in UpToDate including graded recommendations, professional topics, medical calculators, select Lexicomp® drug monographs, original infographics and patient education topics. UpToDate Pathways within Cerner Millennium recommends relevant pathways and UpToDate search terms based on patient contextual information.

Use Cases

  • A primary care physician is treating a 72-year-old woman with a history of chronic kidney disease, hypertension, and diabetes who has a new diagnosis of atrial fibrillation. Using an UpToDate Pathway, the PCP determines that her stroke risk is higher than her bleeding risk and that warfarin is the preferred anticoagulant for her.
  • A nurse practitioner has a telemedicine visit with a 26-year-old woman who reports symptoms consistent with an uncomplicated urinary tract infection, her second in two months. The NP consults an UpToDate Pathway and determines that the patient is at risk for a multidrug resistant organism and that nitrofurantoin is an appropriate antibiotic for her.
  • A medical resident admits a 67-year-old man is admitted to the hospital with several medical problems, including acute hyponatremia and moderate hyperglycemia. The resident uses an UpToDate Pathway and discovers that, after correcting for the serum glucose, the patient has pseudohyponatremia and does not require additional evaluation.
