These videos bring together simple language, imagery, and analogies to drive engagement and understanding of the implications of Type 2 Diabetes on the body over time. Each video has been summarized into a 1-page infographic to give healthcare providers flexibility in how they provide this education to patients. Healthcare providers will be able to access these resources while in the patient chart to use with their patients in the office or send them to patients via email or text.

Healthcare providers can select resources by clicking on their corresponding check boxes or by clicking on the title, and can share with patients by clicking on the Share button. Click the play button to preview video resources and click the download button to download infographic resources.

Lilly Diabetes Share
The patient’s email and phone number are pre-populated from Cerner and the healthcare provider can edit the phone number or email and send links to the patient’s email or as an SMS to the patient’s phone. The Share screen also generates a QR Code link that the patient can scan.

Once links have been shared with a patient, a patient mode of the app will open. Once the patient has confirmed their date of birth, they can view the assigned and shared resources.
