findhelp Launch

Clinicians use findhelp (Aunt Bertha) to connect patients to community social services and track outcomes, positively impacting patient health and reducing readmission rates and healthcare costs.

Customers use findhelp technology to power their social care systems. Our tailored solutions are built on the industry-leading findhelp network which features more than 550,000 program locations that provide help to millions of people. The findhelp application for Cerner allows users to proactively address SDoH factors by connecting and referring patients to free or reduced-cost social services. When users launch the findhelp web application, they are brought to a ZIP Code-specific search; in addition to searching for relevant programs to address the patient’s social care needs, users can make referrals, track outcomes, and document patient goals, and more.

Use Cases

  • Patient has non-medical needs, like food insecurity, housing insecurity, lack of transportation, and more. Care team finds available social services to meet these needs and refer patients to them electronically. The service can be used at any point during the clinical visit, including intake, treatment, and discharge. Population health departments can better understand the broader trends of their patients’ social care needs.
