CancerNAV

CancerNAV supports navigator management of the oncology patient population from diagnosis through treatment and increases navigator efficiency, reduces time to treatment, minimizes barriers to care, and decreases patient migration.

CancerNAV is a patient relationship management (PRM) solution that supports efficient and timely patient engagement, communication and adherence to the treatment plan. CancerNAV increases navigator capacity through specialized navigator workflow and tracking of care coordination activities with a streamlined dashboard, disease-specific checklists and color-coded progress benchmarks, reminders, and alerts. Access to care and patient retention is improved through identification and resolution of barriers to care decreasing time to treatment for newly diagnosed oncology patients. Quality improvement initiatives and documentation to accreditation standards are further enhanced with actionable point of care metrics, allowing oncology programs to demonstrate value, success, and sustainability.

Use Cases

  • Patient contacts the Cancer Call Center stating that he has recently been diagnosed with lung cancer and needs an appointment for a second opinion. The Navigator adds a New Patient and diagnosis in CancerNAV then follows Lung Cancer Checklist to ensure timely access to care.
  • CancerNAV standardized metric reports are reviewed by the Oncology Services Manager to identify the number of new patients and internal referrals, and the time-lapse from diagnosis to surgery. Automated point-of-care metrics available in real-time saves several hours of manual data entry and provides metrics needed to assess patient- and system-related barriers to care.
  • Navigator receives an Alert in CancerNav that the date of surgery falls outside of Time to Treatment benchmarking. The navigator notifies the clinical team and the surgery date is changed to ensure timely access to care.

Key Features

Navigator Dashboard

Disease-specific checklists in CancerNAV efficiently guide navigator workflow, fostering patient-provider and inter-team communication to ensure timely access to care. On the navigator dashboard, alerts and notifications identify and assist with the coordination of care. Real-time documentation of encounters using customized workflow selections, bench marking to key treatment milestones and point-of-care metrics have shown to improve patient experience and clinical outcomes.

Patient Record View

Documenting care coordination in CancerNAV, such as tracking towards key treatment milestones and addressing barriers to care has demonstrated improvement in Time to Treatment by 30% and early identification of outliers, resulting in adherence to treatment plan. As navigators address individual patient needs and provide customized support, education and resources, patient engagement and retention drives additional revenue to health systems. Documented reduction of 26% in patient leakage results in significant cost generation for billable services and cost savings by streamlining the process of pathway adherence.

Cerner EMR Integration

CancerNAV integrates seamlessly with the Cerner EMR eliminating duplication of data entry and ensuring that navigators can efficiently coordinate care for a rapidly growing patient population. CancerNAV increases the ratio of navigator to patient by increasing the navigator productivity by 61%. CancerNAV allows you to prioritize care for high-risk patients across the care continuum and build capacity within your cancer programs, allowing clinical staff to focus on “top of license” activities.

Access to the Patient Record

CancerNAV is embedded in the Cerner EMR providing navigators access to additional real-time patient information such as scheduled appointments, other diagnoses, problems lists, and medications. This supports care coordination for Time to Treatment and improved patient outcomes as you strive to improve the quality and timeliness of cancer care to patients.
